Fax your CME credit forms to (312) 202-8202, Attention: Division of CME, or email attachments to cme@osteopathic.org. Be sure to include your name and AOA ID.

Reporting and Non-Osteopathic Forms
Home Study CME
  • Home Study Form 2013-2015
    This form is for individual DOs to document reading of recognized scientific journals, listening to approved audio tapes, and other approved home study courses and programs under the criteria described for Category 2-B CME.
  • Certification of Home Study 2010-2012
    This form is for individual DOs to document reading of recognized scientific journals, listening to approved audio tapes, and other approved home study courses and programs under the criteria described for Category 2-B CME.
Healthcare Facilities CME
Sponsor Forms
  • Roster of Attendance
    Submit this form to report and designate CME credits for DOs who have completed CME programs sponsored by your organization.
  • Roster of Specialty CME
    This form is for reporting and designating specialty CME credits earned by DOs who hold specialty certification and have completed CME programs sponsored by your organization.
Waiver of CME Requirements
  • CME Waiver Form
    You can use this form to request a waiver or reduction of your AOA CME requirement due to mitigating circumstances.
  • Missionary Form
    This form is an application for possible exemption from AOA CME membership requirements and possible reduction of membership dues during time of missionary work.
