The deadline to apply for an EHR Meaningful Use hardship exception has been extended to July 1, 2016. See instructions below for completing the application.
Step 1: Download the application. Type directly into the PDF to add information, or you can print it out and complete by hand.
Step 2: Provide contact information in Section 1.1 for the person applying on behalf of the physician or group. For example, for a solo practitioner, this can be the physician or a staff person. Employed physicians or those in large group practices should confirm with their administrators if they will be applying.
Step 3: Select the hardship category from among Sections 2.1 to 2.4, and fill in the name of the same contact person provided in Section 1.1. The AOA suggests selecting category 2.2.d, which notes CMS’ delay in releasing modifications to the EHR Meaningful Use rules. The category selected must apply to all the providers in a group application.
Step 4: Skip Section 3. In Section 4, enter the NPI as well as the first and last name of the physicians being applied for. If submitting on behalf of multiple physicians, fill in the total number as well. An electronic file, such as an Excel spreadsheet, can be submitted instead if easier for a large group.
Step 5: Confirm, enter date and type in name of the individual from Section 1.1 into the provided space.
Step 6: Save a copy of the file for your own records. Then submit as an attachment via email to firstname.lastname@example.org, or fax it to (814) 456-7132. Email submission is greatly preferred.
