On the Frontlines with Alzheimer’s Disease: Diagnosis & Management

Purpose

Primary care physicians are on the front lines of identifying and diagnosing patients suffering from varying forms of dementia. It is imperative that they are provided with the knowledge and the tools to optimize the care they provide to their patients.

Program Outcomes

  • Provide a physicians' resource toolkit containing current practices, tools, and resources useful for diagnosis and treatment as well as the navigation of the health care system to obtain necessary services. 

  • Provide a self-screening tool for patients/caregivers to facilitate early conversations.

  • Evaluate the education program, resource toolkit, and the self-screening tool to assess any changes in physicians' perspectives and behaviors regarding Alzheimer’s disease and other types of dementia.

Overarching Goal of Toolkit

The primary goal of the toolkit is to foster patient communication and utilize physician resources to address dementia, with a focus on Alzheimer’s disease.

The toolkit outlines:

  • Needs and benefits of earlier detection.

  • Knowledge of diagnostic tools.

  • Resources.
