AOA Teamwork to Advance Medical Home Education 

I represented the AOA on 3/30/12 at a meeting of the Patient-Centered Primary Care Collaborative’ s (PCPCC) Board of Directors, on which I hold the AOA-designated seat after concluding my term as Chair in October 2011.  Having recently hired a new Executive Director and Chief Operating Officer, the PCPCC is undergoing a strategic redesign and planning process to better align its operating structure to support nationwide implementation of and education about the patient-centered medical home (PCMH).  To this end, the PCPCC is hosting a Webinar, “The Medical Home Experience: Care Coordination and the Patient's Role in Shared Decision Making and Team Communication​,” on Friday, 4/6/12.  The webinar will discuss key ingredients of population health and disease management that support the PCMH as well as key success factors in the PCMH.
